Reports to get check-in data for specific terminated employees

Summary:

We have a legal request to provide information contained in Check-ins for a terminated employee. At this time, there is no way to access this information through the UI. I've tried to create a report to pull the information but am not having much success.

Does anyone have a report they could provide in how to get the data out of the system?
