In OTM, how can we customize the Spot rate page in Quote's Cost options page ?

This is the screen which we accessed through the Quote in the Cost Options tab, when we create a spot rate the above screen is shown. I want to customize this screen. Also, how we can customize the Cost option screen in Quote where I need to remove the equipment field from it
