Performance Goal is failing once it is submitted to manager

Content (please ensure you mask any confidential information):

  1. The user has a Line Manager under Organization Chart
  2. He submitted the goal to that Line Manager
  3. However it is not being assigned to that manager, and, it is failing in the transaction console - anyone knows WHY? and HOW to fix?

This seems to happen only for this user and Line Manager. thanks a lot in advance
