Is it possible to allow only the worker to complete a questionnaire and manager only view response?

For part of our evaluations we would like to have a questionnaire for the worker to fill out. It is one question. Then once the eval is passed to the manager, we only want the manager to see the question and answer. The manager does not need to take action, only see the answer given.

I have the template set up, and the worker sees the questionnaire and answers. Then when it gets to the manager, they see and have to answer the same question which we want to eliminate. Only want them to view the worker response.
