How to make Questionnaire field default in Feedback page.

We received feedback from the client's leadership stating that the feedback page in Redwood is not intuitive and provides a poor user experience.

To address this, we would like to set the Questionnaire field to default to the only available questionnaire and remove the Request Feedback From field, as this field currently allows selection of anyone. Could you please suggest how to hide it?
