Process flow task are blank in performance document and the flow is shuffled in document period.

We have created a process flow and attached to 6 performance documents, out of which the 2 documents the task are blank and under document period the process flow is shuffled. Has anyone encountered the issue, if yes how did you resolve it. Also please confirm if it will have any impact on the employee's performance document.
