In a Compensation Change Statement, how do I get the header to display on subsequent pages?

Summary:

In my RTF template for a Compensation Change Statement, I have a header, using the actual header functionality inside Word. My hope is that the header would display on each page, just like it does in the template. However, when generating a Compensation Change Statement or Preview, the header is ONLY displaying on the first page.

Does anyone have guidance on how to make the header display on each page of the statement? I'm attaching two screenshots below to help understand my issue. Screenshot 1 is of the RTF template, where you can see the header is correctly displaying on each page. Screenshot 2 is the output, missing the header on Page 2. (Please excuse the masking I had to do of company logo)
