Extensions not firing intermittently

Summary:

We have a few Extensions in our implementation. They are fine , logic is working. But time and again (intermittently) these dont get triggered, as the fields we set through the extensions dont get set.

What could potentially be causing this?

What are the measures we can take to diagnose and knock them off?
