Low Pulse Survey Response Rate Nudge Not Working

Summary:

I have set up the configuration for "Employee has a low pulse survey response rate" as shown below. The nudge is still not triggering properly. I have tested this by assigning a pulse survey to the same employee 4 times, although we have it set to trigger after 2 times. Am I missing something because the nudge is still not triggering?
