Performance Evaluation Questionnaire section is not shown when printing

Summary:

Questions/feedback for our Questionnaire section of our annual performance evaluation is not shown when printing as the employee or the manager at any step or after final submit. How can we get this questionnaire section to print? Currently, it only shows the names of the manager and the employee. Is this something that has to be modified within the .rtf?
