Getting an error "System fault encountered in Human Workflow" in Global Absence managemnent

Hello,
For our customer requirement we have created one rule for Unpaid Leave under Global Absence Management- Single Assignment rule. However we are getting an error during AOR approval as "System fault encountered in Human Workflow".

Can someone help with the issue resolution?

Below is the screenshot of the approval rule.

First two represntatives rules are working, we are getting an error for the third representative(Divison head).
