Incident Reviewers/Approvers: When an incident is rejected, can we add comments

Summary:

Currently, when an Incident Reviewer or Approver rejects an incident in the Health and Safety module, the system does not provide an option to add comments. Because of this limitation, end users do not know the reason for rejection and need to follow up outside the system, which reduces efficiency and transparency.
