Unexpected Rating Displayed in Performance Document Questionnaire

Hello,

While reviewing their report’s performance document, managers noticed that the questionnaire section shows a rating of 0 for both the Manager and the Employee (please see attached screenshot).

However, scoring is not enabled in the questionnaire configuration, so no rating should be appearing in that section of the performance document.

If you have encountered this issue before, we would appreciate it if you could share the root cause and how it can be resolved.
