Does anyone document Refusal to provide Emergency Contact Details?

Summary:

We have interesting situations in some of our global countries where employees can refuse to provide Family and Emergency Contact Information. Unfortunately, because the information is left blank, and it is important from a Safety standpoint that we have it, we have HR following up with them on a constant basis to have this entered, when they have already refused several times.

I would like to know if you have experienced similar situations, and how to do you handle documenting the refusal from the employees in providing emergency contact information?
