Hello - hoping some can help with this one that feels doable and easy yet I am struggling!

Using Feedback Centre for ANYTIME FEEDBACK

Can you create a category list and or a flexfield when giving feedback only?

For context we want to select the type of feedback it is for example is it "Positive" or "Constructive" (mandatory) and I want to optionally select

"Which 'Expectation' is this feedback related to?" LOV that contains items such as below:
