How can we create custom checklist alert templates which includes DFF values?

We have a offboarding checklist requirement, We have created configurable form where we are capturing few inputs from managers using DFF's.

once manager completes the task, owner should get notification along with captured response from manager.

Can we create custom alert template which captures DFF values and display it in fyi to the owner?
