EHS Assignment Rules Not Working – Only First Rule Executes.

I have configured an EHS Assignment Rule Set with multiple rules (Stakeholder, Safety Coordinator, Approver, Reviewer, Owner) using the same incident value conditions. The rule set is prioritized first.
I created custom Safety Coordinator roles with all required Function Security and Data Security, assigned them to users, and ran the Import User and Application Security Role Data process. Added User to delivered EHS Manager role. Alert Composer notifications are enabled.
Issues
- Only the first rule fires; all other rules are ignored.
- Splitting rules into separate rule sets produces the same result.
- Removing conditions still does not assign all expected users.
