HDL for loading worker Check-ins - including prompts

Hey guys - moving from a third party system to Oracle for Check-ins and we want to map and load in any existing Check-ins into Oracle for historical purposes. The prompts which in Oracle we use questionnaires and I can't see a field for this in the objects, can anyone help please with a working template!
