Benefits summary page shows incorrect number of pending actions

We've noticed that the number that shows up in the bar with 'pending action items' in the benefit summary page is not always correct. When testing 25D, there is one person who shows that there are 9 Pending Actions, but when you click the 9 there are actually 12. I ran an OTBI report and it shows the same 12. Has anyone else noticed that, or know why we are seeing this discrepancy? We are still in the responsive UI, so maybe it will be corrected in Redwood? It only happens occasionally, but I am curious if others have noticed it or know why.
