"New Goal" Page Not Loading

Summary: When delegates are trying to add a new performance or development goal for someone, after they select the "Add Goal" radio button, the new goal page never loads - just has a spinning circle. We have looked at a number of FSPs, DSPs, and inherited roles and have been unable to resolve the issue. Hoping for some guidance on other things we can look at.
