Pet Care Plan not returning monthly employee contribution

Summary:

Customer want a drop down instead of separate tiles for employees to pick from the number of pets they want on the Pet Care Plan. Created the Coverage with a Flat Range of 0 to 15. Attached a rate valuation fast formula to the Standard Rate. When employee selects a '2' for the number of pets. The employee contribution and annually fields remain at $0. Ran the Evaluate Formulas and the process returned the expected amount of $34.00.

Since this did not work or we have something not quite configured. Our 2nd attempt involved creating Variable Rate Profiles - no success.

Expectation: Employee selects the number of pets from the drop down and the Employee Contribution and Annually fields are populated accordingly.
