Inconsistency in All-in-one evaluations page

When managers use the all-in-one evaluations page to provide an evaluation for their team and click on submit, the documents picked for submission is inconsistent.

Content (please ensure you mask any confidential information):

For example, we only have the final rating as a required field and everything else in the entire performance document is optional. So if a manager provides ratings via the All-in-one evaluations page for the employees (say ratings are provided for 3 out of 5 employees), and clicks on submit, only 2 employee's documents are 'completed'. If i click 'Submit' again, the 3rd employee's document is also completed (although i did not do any action on the employee's data).
