Benefit Plans are Duplicating

When an update was made and saved for one of our Plans, it duplicated the Plan. Below are the screenshots of what happened.

This screen shot shows what the list of plans looked like prior to the update.

I made an update on the Enrollment train stop.

And I made an update on the Certifications train stop.

Once I saved my updates, I now see duplicate Plans for Officer Life on the search page.

In addition to the duplicate plans. I have duplicate Plan and Options.

Any idea what might be the issue?

We recently had the 26B release applied.
