Employee Development Goals not getting Saved in Redwood Goals Center page

Summary:

Hi Experts,

In ESS , Me > Career & Performance > Goals Center, when an employee Adds a Development goal (from Development Tab) after selecting a Learning Item or a Task and tries to "Save" ( button at bottom-right), it does NOT work or gets Saved.
Also the "Save" button at Top-Right getting grayed out to submit the development goal.

Please advice what could be the issue?
