Rejection Reasons - several questions here

Once you've identified a candidate as not qualified either after phone screen, interview or what not - how are people tracking the reasons why they're rejected. I have the prompts set up to select a rejection reason, but is that the only place? If so, how do you pull a list of of those rejected and why (maybe keeping track of when salary is not in line, etc). Or would you add to the comments section? And if so, are there any implications that could come back to you by having comments in that section?
