Performance Reviews "Re-Push"

How does everyone manage the process of pushing out a second/third/fourth wave of performance reviews for employees who fail to have goal plans finalized by the due date? Since there will not be editing on goals after the performance reviews are created/pushed we anticipate a number of requests to "re-push" reviews. Was hoping to get a high level overview of some processes in place out there today beginning from employee/supervisor all the way through to the system administrator.
Do you do this on a weekly basis? biweekly?
What are the approval steps put into place to allow this exception?
