Dropdown options for Onboarding Forms

We use several of our onboarding forms to enter information into our HRIS.  I created the pdf to have dropdown options, but it appears they are not supported when they are uploaded.  Am I doing something wrong or is there another way to add dropdown options to the forms?

Any help or suggestions are appreciated!
