performance appraisals without ratings

I have been asked by senior management to create a performance appraisal template that does not include ratings (i.e., givng someone a score). They would like me to include the various competencies with a comment box for the manager to state how the employee is doing on that compentency, but they don't want managers to have to select a score (i.e., 1-5, 3 stars, etc.). Any ideas on how to make this work?
