add embedded template to all reqs and be able to save information to it

Hello,

   Trying to add a work document template for a strategy session  for each req.  This document will automatically appear on each req as it is created.  The recruiter will be able to open it and add information to it and then save it again to that same req.   Please advise on how this may be completed.
