Trying to develop pipeline/funnel report

Does anyone already have a report in place that provides historical counts of candidates in a requisition such as:  resumes received, phone screens completed, interviews completed, offers made, etc.  I need a report that I can run monthly to capture the funnel of candidates.  I am having a lot of difficulty with the historical measures in reporting and keep getting bizarre numbers returned that do not match anything.  I have opened an SR but so far no luck in getting help to fix the report.
