Blank PDF

I assigned a review to an employee and required an employee self-assessment. I wanted to be sure that on the final PDF, the employee and the user were able to see both scores. I doubled checked my page layouts allowed for this. Once the review was finished, I clicked on the employee profile to view the PDF and it is coming up blank, but I can view the scores within Taleo so I can see that both parties submitted scores. Any idea why the PDF is blank?
