Assigning forms to a User Activity

Hi,

Has anyone had success in assigning a form to a User Activity? From what I can see, it appears as though forms can only be assigned to an Employee during Onboarding - but wanted to check if I am missing something.

Any information or suggestions you may have will be appreciated.
