Has anyone added the new OFCCP disability/veteran form to their app flow yet?

Hello, we are getting ready to implement the new requirements to collect disability/veteran information pre-offer and I have read all of the existing documentation re: the new form/PDF that was introduced to collect this data - just trying to think through the following:

-Should we gather from ALL applicants during initial submission or consider a second application at a certain step/status?  If you are doing as a second pass, when are you triggering?

-Should we use the PDF form that Taleo delivered, or add questions into existing EEO/diversity block and use answers to populate PDF form on back end?  Any benefits/watch items for either approach?
