Manager Can't Approve Review

A manager created a review and added his manager as the Directly Assigned Approver.  This person (the manager's manager) is also listed against the Employee Record as the Review Approver.  But when this manager selects "Approve" for the review, she receives an error:  Security Problem:  You are not on the list of review approvers for this review.

Clearly something is wrong in the set up somewhere.  Help!
