Importing Requisitions Question

I'm trying to import 200 requisitions into Taleo.  I've follwed the directions, used a CSV files, and everything matches up - However, though I have 201 lines in my files, only one requisition is showing up after I complete the upload.

Any idea what I'm doing wrong?  Also, there is no formatting in the Description I provide, so the whole thing is just run together text.

Any ideas?
