Goal Plan Not Syncing with Review After Submittal

We are having a situation where employees complete their goal assesment and send it to their manager.  Once they send it if the employee makes any changes in their goal plan such as updating the status on a goal, that change does not get updated on the employee's assessment.  Even if the manager sends the review back to the employee to correct something the sync between the goal plan and the goal assessment is lost.  Is anyone else having that problem?  Do you have a solution for it?
