employee esigned wrong review

for some reason, we have two versions of the review showing in the employee website. one is "blank" and one is the manager version. employees are confused and i just had an employee sign the incorrect version and i don't know how to make the other version viewable so he can esign it.
i'm wondering if this is happening because we enabled self assessments but all managers do not require them.
not sure how to handle.
