I hope the review manager didn't click - More and Reset Submitted

Hi team,

For my employee Cheryl Tingler, she and manager and I don't see self-assessment icon in her assigned review.  History says she submitted it 2/5/2016 and she showed me a PDF she created of it.

Do you see anything on this?  Is there a record of review manager clicking More / Reset Submitted, the only way I know to make the icon in the review disappear.
