I need to report on Offer Dates

I would like to create a report on offer dates and am struggling with how to do that.  I am trying to report on what the Offer Created, Offer Extended, Offer Accept, Offer Start Date, Hire Date, so I can see any Start Date Updates.  Here is what I see in the candidate file:

Offer Created Offer Extended Offer Accept Offer Start Date Hire Start Date
2/15/2016 2/15/2016 2/15/2016 3/14/2016 3/21/2016

But when I pull in the Offer Actual Start Date field, it shows the Offer Start Date as 3/21/2016 because someone made a Start Date Update from 3/14 to 3/21.  I need to be able to report on the two different dates.
