We would like to add a configurable form to interview feedback

We would like to use a standard feedback matrix/form for interview feedback by hiring managers.. So feedback is consistent and can be viewed across the Requisition within TBE..

I think I can put all the pieces together to create the custom fields for the form, and place the right field merge code into my form, but I can't seem to sort out where I can add this form..

Within Onboarding my Configurable Forms page allows me to "Upload PDF Forms", but I am not seeing this within Recruit..
