Easy opt-out of otherwise mandatory Diversity Questionnaire

I've been asked to explore solutions to make responses mandatory for those who opt to complete the form, but non-mandatory for those who wish to skip it.
In a nutshell:
1. Do you wish to complete this questionnaire? Y/N
- If Y: An answer must be provided for all subsequent questions
- If N: Subsequent responses are not collected / not mandatory
It's insufficient to send a follow-up email or invite them to complete a survey elsewhere; this has to be integrated into the application process.
