Questionnaire with "No data to display"

What does it mean when the questionnaire says "No data to display"?

I have Recruiters running into situations where a job submission has the little caution sign that someone has answered a question other than optimally and to investigate further, however when they get to the Questionnaire section, they see "No data to display".  In 3 1/2 years I've never run into this.  Has anyone else seen this before?  Everything in the Permissions looks right.
