Creating custom forms with custom fields for a custom tab?

I have the custom form created. Also custom tab and custom fields. But can't get the form to show up on a custom tab.

I would like this form to be submittable to a workflow for approval. Just can't get past this part.

Any ideas? The user guide does a great job of explaining each part but does a poor job of explaining the connection of forms to tabs to application views.
