Help With Onboarding Formula

I was hoping someone could help me create a formula. Every time I try, I keep getting duplicate rows for each candidate so I am obviously missing something.

We have a Pre Hire Process that contains 3 Task including an Offer Letter task. When all tasks have been submitted, the system creates the Offer Letter PDF. If the candidate does not complete all tasks within 5 days, the process automatically completes without a PDF and the recruiter is notified that they can reject the candidate.

What I want is the following:

A list of candidate names who had the pre-hire process started and then I want a column to show a 'Completed' if that particular Offer PDF task was created, else Not Complete. I tried to use the below but it seems to show Completed and Not Completed for each candidate in my list. What am I missing, or is there a more efficient way of accomplishing this?
