Qualification Questions - how to easily find the 'To Be Verified'

Wasn't there an icon that used to display behind the Qual Question that was 'To Be Verified'?

When a candidate answers one of our Qualification questions in a manner that doesn't meet our qualifications, the candidate is flagged with a Yellow triangle icon above their name on their job submission.  This yellow triangle icon is a notification to the recruiter to check their qualifications.  I thought there used to be an icon located next to the question that needed to be verified.  I'm thinking that with one of the recent upgrades we had, this icon was removed, because we no longer see this on the question that needs to be verified.  Now the recruiter has to look
