Needed Sample Default Enrollment Fast Formula

We need a formula which will default employee into new plan in same option of previous plan and forward dependentsContent
We are changing dental carriers. We need a formula of type "Default Enrollment" which will default enrollment into the new plan if the employee was enrolled in the old plan. The default needs to put the employee in the same option and forward the dependent designation over to the new plan and option.
I've tested the sample formula that Oracle has provided and it is not working. Does anyone have a sample that has worked in the past. In addition does anyone know if we need to change contexts such as effective date to first put the previous enrollment in a local variable.
