How do diversity forms display for requisitions with more than one location

We are thinking of creating a diversity form per country. If we do this and then create a requisition that has locations in addition to the primary location what does the candidate see? Will they have to fill in every diversity form for every country to which the requisition is associated?
I looked on My Oracle Support and found document 1045948.1 which seems to indicate that is the case but has anyone had experience of actually doing this?
I also raised a ticket with on My Oracle Support and they suggested I test it which I can do but we haven't actually decided whether we will do this yet and wanted this to form part of our decision.
