Facing Issue with Delivered Worker Extract - In Changes Only Mode

Hi Team,
I am trying to work on delivered worker extract in R11 as well as R12.I have copied the delivered extract and followed the Oracle documentation as "HCM Extracts: Seeded Work Structure and Worker Extracts".
I have hired one employee and submitted the extract in with "ALL ATTRIBUTES" mode and was getting all the field.
After that I have updated the name details for the person as well as salary details added one more row.
When I submitted the extract in "Changed Only" mode ,I am still getting all the records means all person record as well as two row for person name with old and new value as well as two row for salary old and new value.
