Getting error unable to process screening request

I'm getting an error Unable to process screening request when I try to Request Background Screening for a candidate

Hi,

We have integration with GIS from Taleo recruiting for Background check. When I go to a candidate profile > More Actions and select Request Background Screening. After selecting the agency and clicking 'Done', I get error message 'Unable to process screening request'. 

Any idea why I must be getting this error?
