Candidate Application Icon Missing

For some candidates who have applied to many jobs, the application icon does not show for all reqs

How can candidates apply to positions without the application icon showing next to the requisition (see screen shot below). The individual with no icon in the circle applied to almost 20 jobs, and about half of them have the icon and the other half do not.

Additional question, why do the job alerts that go out to a candidate get logged using my user name? (second screen shot)
